Accountable Communities for Health (ACH)

An Accountable Community for Health (ACH) is a structured alliance of healthcare, public health, and other sectors that plans and implements strategies to improve population health and health equity. ACHs are designed to leverage resources across organizations. The ACH model offers opportunities to reduce costs, enhance quality of care, and improve population health.

The ACH model is used by initiatives across the United States, some of which have been studied by the Prevention Institute.
